We are here to help you connect the dots and find your new ‘normal’ whilst living with cancer. The focus of the COUCH Wellness Centre is on health and well-being to enhance quality of life across all areas of practical, physical, social, emotional, cultural and spiritual wellness. Our care partnership program provides the supports you may need, at the right time.
If you have recently been diagnosed, in recovery, survivorship or palliation, the COUCH Wellness Centre is here to support you during your experience with cancer. We welcome all enquiries and love visitors! We encourage all potential new clients to drop into the COUCH Wellness Centre to complete a tour to find out about our services. Our care partnership program is also available to carers, close friends and family who are impacted by your cancer diagnosis.
Whilst we prefer your GP and treating team to refer you to our service, we also welcome you to make direct contact with us about your needs. If you chose to join as a client, we will require verification of your diagnosis from your treating team or GP at the time of joining. We can help you to arrange the appropriate documents required. All clients must be within 5 years of their cancer diagnosis to access support and services at the COUCH Wellness Centre.
Our care partnership program means we can link directly with your GP or treating team to provide the very best individualised care for you. This is known as shared-care and research shows this style of integrated cancer care promotes optimal outcomes.
When you first choose to contact us or drop in to the Centre, we will provide an overview of how our service may be able to support you. We suggest arranging your visit when you have time to explore all that is available at the COUCH Wellness Centre. During your visit, one of our amazing team members will show you around and answer any questions you may have.
You don’t need a referral to visit us. We welcome all enquiries and your family are welcome to join too.
If you feel the COUCH Wellness Centre is a good fit for you, just download our care partnership form or request a paper copy from Reception. Once you have completed and returned this form, one of our team will connect you with your cancer care nurse for your initial assessment.
During this meeting, we listen to your story, explore your needs and provide information and resources, plus answer many of the questions you may have. We can help you to identify the little and big things that you may need support with and start working with you to find solutions for your health and well-being.
Our care partnership program helps puts the pieces of the puzzle together. Your cancer care nurse assists you with navigating your care needs during and after treatment. Together, we will create a personalised Wellness Care Plan, which may include referrals to health professionals, supportive therapies, group sessions and classes, plus any lifestyle supports which may be of benefit to meet your needs.
Your cancer care nurse is right there with you during your experience with cancer. Your pathway to wellness is determined by you, at your own pace, to meet your needs. We provide the supports and services; you decide what works best for you.
The COUCH Wellness Centre provides a hub for health and well-being, it creates a space for people diagnosed and living with cancer to access services all under the one roof and creates networks to all other supportive services within the region.
